• Any recent notices you’ve received regarding changes to your health insurance
• Proof of income, such as:
  • Last 60 days of paystubs
  • Statement showing retirement income, disability income, worker’s compensation, unemployment benefits, etc. (if applicable)
  • 2024 tax documents (W-2 and/or tax returns if you filed taxes for 2024)
  • 2025 tax documents (W-2 and/or 1099s if you have them)
• District Direct login information (if you have an account)
